What anti-spam features do you have enabled on your forum?

Does it block all spam?

If not, what percentage of spammers are you blocking?

It is easy to block bots. But human spammers are persistent. Drives me mad sometimes.

Mainly CAPTCHA. It is very effective.

Modern-day CAPTCHAs are working fine for me. It is the persistent human spammers that are causing the issue.

I used to use Security Questions but they seem like they don't work good anymore.

Now I've been using ReCaptcha and it seems to do better!

Even recaptcga isn’t foolproof..I use Ozzys registration spammanator..I have use it sence the vbulletin days..

hCAPTCHA is also nice.

Security questions are good too. But not with human spammers. Making life difficult for them in effect affect the forum activity.
